We are a faith-based company, dedicated to offering superior in-home care to your loved one by providing skilled and experienced caregivers. We provide quality care, in the comfort of your own home.


We can provide live-in or hourly non-medical home care to seniors, disabled individuals and Veterans who need extra assistance to live comfortably within their homes. Care can also be received in hospital and/or skilled-nursing facilities.


We carefully screen our staff with extensive background checks as well as diligent reference checks.

Meet The Founders

Antoine

Antoine is the owner and founder of PC Home Health. Having a personal experience with family members that have Alzheimer’s/Dementia, as well as experience as a Certified Medical Assistant in a geriatric facility, he has made it his mission to serve others and navigate this hard time with so many families. His love and compassion for others is what drives the vision of PC.

Bryunna

Bryunna, also known as Bry, is a Certified Medical Assistant and Certified Dementia Practitioner. She has worked in various fields, from Geriatrics, Internal Medicine, Endocrinology, OBGYN and Plastic Surgery. Training and improved quality of care for client’s is her passion. She is the Alternate Administrator, here at PC, and loves every minute of getting to know the families we get to meet on a daily basis.

What are the Steps to Working with PC Home Health?

01. Give Us a Call

Are you needing extra assistance with your loved one? Do they need help with transportation to appointments, meal preparation and medication reminders? Then give us a call, we can help!

06. Routine Check Ups

Just because care has started, doesn’t mean we aren’t interested in the care being rendered. You will see our staff often as we do our “Routine Check Ups” on our caregivers and clients. We’ll also want to hear your feedback as to how we are doing and if changes need to be made.

05. Care Begins

Once a caregiver is matched to your case, the care can begin. The caregiver will perform the tasks that were approved on the Plan of Care/ISP. Then you and your family member can enjoy having someone there that is in your corner and loves you like family!

02. Set Up a Consultation

Once you have placed a call, you will speak to one of our representatives who can help set up a consultation. Consultations are FREE and are done at the client’s home. A consultation should take about an hour and are done to help us get to know your loved one and their needs better.

03. Create a Plan of Care/ISP

During the consultation, a Care Coordinator will discuss with you and your loved one about what your needs may be and evaluate if we can meet those needs, or if there are more needs that would benefit you. From there a Plan of Care or Individualized Service Plan is made specifically for the client that will detail what services our caregiver will provide for the client, on what days and at what times. We also discuss our specialty programs to see which is a right fit for you!

04. Find/Match a Caregiver

Bringing a caregiver into your home is an important responsibility of ours, and because of this, our Caregiver and Client Matchmaking is super important to us. During the consultation, we get to know you and your family thoroughly so that we can match our caregiver to your needs.
